Individual Scholarship Request for Payment

In order to receive scholarship funding, completion of this form is required for approved Patrick F. Taylor Foundation Scholarship Recipients. Before beginning the form, you must have your final transcript and class schedule to upload. All fields need to be filled with accurate information.

Beginning with Fall 2020 semester, new scholarship recipients are required to earn a 2.3 GPA (Grade Point Average) at the end of the Freshman year. Thereafter, scholarship recipients are required to earn 24 hours each academic year and maintain a 2.5 ‘cumulative’ GPA (Grade Point Average) after earning 48 hours.
